Merit badge 2010 Selection Form
8th Grade + older Adventure Form
Health Form Requirements
BSA has introduced a NEW national typeable/signable Health Form A, B, & C (replacing Class 1, 2 & 3 health forms).
Parts A & C are required for all scouting activities.
Part B, physical exam, is required for all ages annually if staying > 72 hrs at camp.
All forms A, B, & C are to be completed, dated, signed and vaild for 12 months only.
