Class 3 Health Forms For Parents over 40
Health Form Requirements
Class 1 Forms - Must be signed each year.
Class 2 Forms - Requires physical exam and Doctor's signature.
Class 3 - Required for Parents over 40 - physical exam yearly
Scouts - Form must be less than 3 years old
Parents - Required if attending Camp for more than 72 hours
